چکیده

Coronary artery anomalies are uncommon, with an incidence ranging between 0.2% and 1.4%. Among those least frequently observed is left main coronary artery atresia. We report the case of a 72-year-old man, with asymptomatic severe aortic stenosis and moderate ventricular dysfunction. He underwent preoperative coronary angiography in which it was not possible to examine the left main coronary artery. Right coronary artery was dominant and the conal branch had an independent origin (Figure A and video 1 of the supplementary material). Blood was delivered to the left coronary tree by way of collateral circulation arising from the conal branch, which was anastomosed to the segment proximal to the anterior descending artery (Figures B and C, asterisks; video 2 of the supplementary material). The anterior descending and circumflex arteries were connected at their origin in the base of a blind pouch (Figure B, arrow head). There was no evidence of significant coronary artery calcification or stenosis. High-resolution computed tomography was carried out to ensure complete evaluation of the anatomy. Three-dimensional reconstruction revealed left main coronary artery atresia and absence of the left coronary ostium in the left sinus of Valsalva (Figure D, arrow head), and confirmed the normal position of the anterior descending and circumflex arteries. The double-barreled origin of the right coronary artery and the conal branch and their anastomosis to the anterior descending artery were also clearly visualized (Figures E; Figure D, asterisk). Left main coronary artery atresia was confirmed during valve replacement surgery and, 1 year later, the patient remained asymptomatic and with normal ventricular function.

Congenital atresia of the left main coronary artery associated with patent ductus arteriosus and aortic regurgitation.

Congenital left main coronary artery (LMCA) atresia is a rare anomaly and surgical revascularization is recommended once the diagnosis is confirmed. In some cases, LMCA atresia will coexist with other cardiac anomalies, mostly in pediatric patients. A 32-year-old woman had congenital LMCA atresia associated with patent ductus arteriosus (PDA), aortic regurgitation (AR), and moderate pulmonary h...

Multiple myocardial bridge in a 59 year-old woman

Introduction: Myocardial bridging is a congenital condition in which a segment of a major epicardial coronary artery has an intra myocardial route. Myocardial bridging is usually confined to a single vessel and is typically the mid segment of the left anterior descending artery. Bridging may be associated with chest pain, myocardial infarction, and sudden cardiac death, or usually asymptomati...

Congenital atresia of the ostium of the left coronary artery. Diagnostic difficulty and successful surgical revascularization in two patients.

We report two cases of congenital atresia of the ostium of the left coronary artery. Case 1: a six-month-old infant presenting with serious cardiac insufficiency. A noninvasive diagnosis of dilated myocardiopathy was established and the clinical picture was pharmacologically compensated. When the patient was nine months of age, a hemodynamic study was performed that revealed congenital atresia ...

Congenital Absence of Left Circumflex Coronary Artery

Congenital absence of left circumflex artery is a rare congenital anomaly of the coronary arteries. The prevalence of the anomaly in different studies ranges from 0.6% to 1.3%. Of these, 80% are benign and asymptomatic and 20% are clinically important. We report a 56-year-old man presented with acute resting chest pain who was diagnosed as having acute anterolateral infarction accompanied by el...

Redo coronary bypass grafting for congenital left main coronary atresia: a case report

BACKGROUND Congenital left main coronary atresia is an extremely rare coronary anomaly. Long-term surgical outcomes and the optimal management strategies for recurrence of ischemia remain uncertain. Herein, we present a case involving successful redo coronary artery bypass grafting for unstable angina 27 years after the initial coronary artery bypass grafting for congenital left main coronary a...
